Helping You Navigate Durable Medical Equipment Coverage

Understanding insurance requirements for durable medical equipment can feel complicated. Vital Doctors Medical Equipment helps patients, caregivers and healthcare providers navigate eligibility verification, documentation requirements, prior authorization and estimated out-of-pocket costs.

We currently accept:

  • Original Medicare
  • Participating Blue Cross and Blue Shield of Texas (BCBSTX) plans
  • Private pay

Coverage is not automatic. Approval depends on the requested equipment, the patient’s eligibility and benefits, medical necessity, supporting medical records, prescription or Standard Written Order, authorization requirements and the terms of the patient’s health plan.

Medicare

Vital Doctors Medical Equipment is a Medicare-enrolled durable medical equipment, prosthetics, orthotics and supplies (DMEPOS) supplier.

Medicare Part B may cover certain durable medical equipment prescribed for use in the patient’s home when the equipment is medically necessary and applicable coverage requirements are satisfied.

Depending on the patient’s condition and Medicare requirements, eligible equipment may include:

  • Manual wheelchairs
  • Certain power wheelchairs
  • Certain mobility scooters
  • Walkers and rollators
  • Certain hospital beds
  • Other qualifying mobility and home medical equipment

A prescription alone does not guarantee Medicare coverage. Medicare may require a face-to-face evaluation, detailed medical records, a Standard Written Order, prior authorization or other supporting documentation before equipment can be provided.

Patient costs may include the applicable Part B deductible and coinsurance. The amount owed depends on the equipment, Medicare’s payment rules and whether all coverage requirements have been met.

Blue Cross and Blue Shield of Texas

Vital Doctors Medical Equipment is an authorized provider for Blue Cross and Blue Shield of Texas.

BCBSTX coverage differs among employer-sponsored, individual, family and other benefit plans. A BCBSTX identification card does not necessarily mean every item is covered or that Vital Doctors participates in every network associated with the card.

Before providing equipment through BCBSTX, our team may verify:

  • Whether the plan is active
  • Whether Vital Doctors is participating in the member’s network
  • Whether the requested equipment is a covered benefit
  • Deductible and coinsurance information
  • Prior-authorization requirements
  • Rental-versus-purchase requirements
  • Medical-necessity and documentation requirements
  • Estimated patient responsibility

The member’s benefit plan ultimately determines coverage and payment.

BCBSTX members may also review their benefits using the telephone number on the back of their member identification card or through their BCBSTX member account.


 

How the Insurance Process Works

1. Contact Vital Doctors

Tell us what equipment has been recommended or prescribed. If you are unsure of the exact equipment name, our staff can help you identify the appropriate next step.

 

2. We Verify Your Benefits

We contact or electronically query the insurance plan to review eligibility, network participation, benefit information and potential authorization requirements.

 

3. We Review the Documentation

Our team reviews the order and available medical records to determine whether the documentation appears to address the applicable coverage requirements.

 

4. We Coordinate With the Healthcare Provider

If additional information is needed, we may contact the prescribing practitioner or referral office to request an updated order, chart notes, face-to-face documentation or other supporting records.

 

5. We Explain the Next Steps

We explain the information available to us, including estimated deductibles, coinsurance or other potential patient responsibility.

 

6. We Coordinate the Equipment

When applicable requirements are satisfied, we coordinate equipment availability, delivery or pickup, setup and patient or caregiver instruction.


 

Documentation That May Be Required

Insurance requirements vary by payer, plan and equipment. Requested documentation may include:

  • Patient demographic and insurance information
  • Prescription or Standard Written Order
  • Face-to-face encounter documentation
  • Medical records or physician chart notes
  • Diagnosis and medical-necessity information
  • Equipment specifications
  • Prior-authorization approval, when required
  • Additional forms required for the requested equipment

Incomplete or inconsistent documentation may delay the order or result in a denial.

Personalized Support From Our Patient Care Team

Navigating a DME order can involve several steps, including insurance verification, medical documentation, communication with the prescribing practitioner and delivery coordination. Vital Doctors Medical Equipment provides personalized assistance through our Certified Caregiving Advocate and DME Coordinator.

Certified Caregiving Advocate

Our Certified Caregiving Advocate serves as a supportive point of contact for patients and caregivers. The advocate can:

  • Discuss your prescribed equipment needs and available options
  • Explain the general steps involved in obtaining durable medical equipment
  • Help you understand common insurance and documentation requirements
  • Coordinate communication with your physician or referral office
  • Explain estimated deductibles, coinsurance and potential out-of-pocket costs
  • Provide updates concerning the status of your equipment request
  • Help coordinate delivery, setup and equipment-use instructions
  • Answer general questions and connect you with the appropriate member of our team

DME Coordinator

Our DME Coordinator helps manage the administrative and equipment-order process. The coordinator can:

  • Review incoming prescriptions and Standard Written Orders
  • Check that required patient, insurance and equipment information is included
  • Identify missing or incomplete supporting documentation
  • Coordinate requests for chart notes, face-to-face documentation and corrected orders
  • Verify eligibility, available benefits and prior-authorization requirements
  • Monitor the progress of pending documentation and authorization requests
  • Coordinate equipment availability, delivery or pickup
  • Communicate order updates to patients, caregivers and referral offices

Our advocate and coordinator work together to help patients understand the process, reduce avoidable delays and keep equipment requests moving forward.

They do not make medical-necessity or insurance-coverage decisions. Final coverage and payment determinations are made by Medicare or the patient’s BCBSTX benefit plan based on eligibility, plan provisions, medical necessity, documentation and applicable authorization requirements.

For Healthcare Providers

Healthcare providers, discharge planners, case managers and referral coordinators may send DME referrals to Vital Doctors Medical Equipment.

Whenever applicable, please include:

  • The patient’s full name and date of birth
  • Current address and telephone number
  • Insurance information
  • Standard Written Order or prescription
  • Relevant diagnosis information
  • Recent chart notes supporting medical necessity
  • Face-to-face encounter documentation
  • Requested equipment and accessories
  • Prior-authorization information, if available

Private-Pay Options

If an item is not covered, the patient does not meet the applicable coverage criteria, or the patient prefers not to use insurance, private-pay options may be available.

Private-pay pricing and availability vary by equipment. Some products may also qualify for CareCredit financing, subject to credit approval and applicable terms.

Choosing private pay does not make an otherwise noncovered item eligible for later insurance reimbursement. Please speak with our staff before purchasing equipment if you intend to seek reimbursement from an insurance plan.

Verification of benefits is not a guarantee of coverage, authorization or payment.

Final coverage decisions are made by Medicare or the patient’s BCBSTX benefit plan. Coverage may depend on:
